when you get temporarly banned or w/e for entering and leaving a game too fast, how long does it take for the ban to go away or end?

Can you only find keys for organs on ladder or is it both ladder and nl?
I have done a couple of runs and havent gotten a single key.

Also can you combine organs on nl and fight uber to key torch ...
